Report a lab issue

If you are experiencing or witnessing an emergency, call 911 immediately.

Use this form for non-emergency problems in the undergraduate labs:

• equipment that's broken, missing or not working right
• a mistake or an unclear step in a lab guide
• unsafe conditions, such as a frayed cable, a spill or a blocked exit
• anything else lab staff should know

Don't use it for questions about grades, deadlines or your lab write-up. Ask your TA or instructor instead.

It helps to say where in the room (your workstation, bench, station or experimental setup) and to add a photo if you can.
